2.  If you have played with either CLYSA or FLSA in the past couple of years you will not need to turn in another copy of your Birth Certificate.  We have it!!

VOLUNTEERING & FUNDRAISING

1.  Lakes United requires that all member families volunteer time to support the activities/events/ administration of the club.  The minimum commitment is 4 hours volunteering per player for the first 2 players per family, for maximum of 8 hours.  This helps to defer a significant portion of the costs of running the Association and provides much needed manpower that ensures our club’s success.  The primary volunteer events are the spring picnic and the June tournament, in addition to various committee participation, fundraising administration, field lining, or other association needs. Participation in fundraising is voluntary but is strongly encouraged and greatly appreciated!

2.  All registering players must also pay an additional $50 volunteer fee ($100 max per family) which the club will hold until August 1st.  Assuming all volunteer hours have been met, Lakes United will not cash this $50 check.  If these hours have not been met Lakes United will cash on August 15th, 2012.

3.  All Volunteer checks need to be turned into your Team Manager or Coach, whether you send in a paper packet or register online.

Player Transfer Release & Refund Policy

PLAYER TRANSFER RELEASE

Any player signing a player registration form to play for LUFC is bound to the assigned team for that soccer season unless the player requests a transfer and the transfer is approved. A request for transfer must be made in writing specifying the reasons and with copies being sent to the coach and LUFC president who will make a determination. This decision can be appealed following the appeal process of MYSA. Player transfers may only be approved for the following reasons:

1. Player relocates to a different community.

2. Voluntary release by club.

3. Player's desire to play at a higher level which is not available at LUFC.

REFUND POLICY

No refund will be given without a written request from the parents to the Coach or Club Registrar.   Refunds:   $50 is considered non-refundable to cover administrative costs.  Any player signing up but withdrawing will be assessed this charge. NOTE:  If a player drops after January 1, 2011, and before March 1, 2011, they will be refunded what they paid minus $50, but if a player drops after March 1 (submittal to MYSA), no part of the fee is refundable.  Refunds for Medical reasons must be submitted in writing and will be determined on a case by case basis.
